Tap the + button in the top-right corner. The editor opens with today's date pre-selected. Set each of the five 1–5 ratings — mood, anxiety, sleep, energy, focus — add an optional note, then tap Save.
They're loosely modelled on items from the PHQ-9 (depression) and GAD-7 (anxiety) self-report instruments:

Tap the entry's row to open it, then tap Edit in the top-right of the detail view. You can change the date, the ratings, and the notes there.
Long-press the entry's row on the main list, then choose Delete from the menu that appears. (No swipe — long-press is the gesture.)
Not currently. If you tap Delete, the entry is gone immediately.

Your data is included in your standard iCloud or iTunes/Finder iPhone backup. There's no in-app export today; if that's important to you, please open a GitHub issue.
No. The anonymous usage signals we send to TelemetryDeck only record that an entry was created/updated/deleted — never the date, never the rating values, never the note text. The only metadata we attach is a single boolean indicating whether the entry has notes at all (not what they say).
